VAC bans up to after 2 weeks after using the cheats. but it doesnt detect everything, aimbots may just work without ban so they have to be manually dealt with.

if you use the stick very often you can gain points even from people with low points since the multiplier is 50. but you have to get a good kpd for that.

valve messed it up with an update in may 2006, since then even if it may look like you hit the enemy you didn´t. also a bad ping of the other player or you leads to misses.
