I noticed this fellow was VAC banned a few weeks back, but it shows he was playing on TSGK three days ago? How is this possible?

It appears to be taking about a month from the initial catch date for a ban to be fully implemented. You will find that the player will be reported as being "caught" each time they play after the first instance and it will appear like a new catch .
It does not necessarly mean that the players is actually cheating every time they play after the initial catch or for that matter that the ban was invoked for a cheating incident in hl2dm.
Basically they can continue playing for a month untill they get fully banned.

Each time he tries to connect to the server it will update the last connected time
even though he was booted for being VAC banned. (If I remember correctly)

Surely a HL1 engine VAC banned player.
HL1 engine VAC banned players can still connect and play on a source engine server like HL2 even on a VAC protected server, so we autokick them and sometime there could be a little delay between the moment they connect and the moment the autokick command is received by the server, this explain the 2 kills he got last 7 days.
