This is cubemap error (missing textures reflection).

It most of the time happend if you minimise the game and come back, then the path to cubemap files (clientside) is lost.

Hi,

Indeed, we don't really know what list of cheat VAC2 is looking, but Valve claim it is 100% accurate and false positives are improbable because each cheat are individualy added to the VAC2 data...
